CMS Special Focus Facility list

CMS publishes two lists, and they are routinely merged into one headline number. There are 440 candidates against 88 designated facilities. Reporting them together overstates the size of the Special Focus Facility program by roughly 5.0×.

The reason the candidate list is so much longer is structural, not clinical: the number of program places is limited. A facility can meet the selection criteria and never receive a place, so the candidate list is not a queue and the designated list is not every facility whose record would warrant it. That limit is also why absence from either list settles nothing — it is a constraint on the program, not a finding about a provider.

These facilities have not been selected for the Special Focus Facility program. CMS lists them as facilities whose records would qualify them for consideration.

• The list changes. CMS adds and graduates facilities as they improve or fail to. This page reflects the CMS release dated 2026-08-21; check the facility page for its own current record.
• The number of program places is limited. Selection depends on available places, so the list is not a complete ranking of facilities with problems, and was never intended to be read as one.
• Designation is not a current-conditions report. It summarises a pattern across past inspection cycles. It does not describe care being delivered today, and no figure here should be read as one.
• No ratings of our own. SeniorCareRating.com publishes no score, rank, or recommendation, and does not describe any provider as best, worst, safe, or unsafe.
