Verify official CMS information
We report what CMS publishes — but CMS is always the source of record. To confirm a facility’s current ratings, inspections, penalties, and ownership, check its official listing on Medicare’s Care Compare and your state survey agency. Our methodology and source dates are on the methodology and data & sources pages.
Report a correction
When reporting an issue, include where possible:
- The facility name, CMS Certification Number (CCN), and state.
- The specific field or statement you believe is incorrect.
- The authoritative source (the CMS Care Compare page or a state survey record).
Because our data is sourced from CMS, the fastest fix for most issues is at the source: corrections that land in the CMS dataset flow into our next refresh automatically. an Illinois limited liability company may review reported issues but does not guarantee a response, investigation, or change except as it chooses or as required by law.
