  • Recent inspections. the findings and dates of the latest surveys
  • Staffing. nurse and aide levels relative to residents
  • Citations. what inspectors recorded, and severity
  • Federal penalties. fines on record
  • Payment denials. a distinct CMS record type
  • Ownership. the CMS-reported ownership group
